Wellbeing and distress in people with an advanced illness
We are looking for participants who have an advanced diagnosis of cancer, kidney, liver, lung or heart disease to participate in a brief survey about well-being. Your contribution will help us to improve the way we support people with advanced illness.

What's involved?
If you choose to participate in our study, you will be asked to complete a survey that will take twenty minutes of your time. You have the preference of either completing an online survey or a physical paper copy of it.
Please either text or email Alesha Wells if you would prefer to complete a paper copy of the survey.
Everyone who participates can choose to go into the draw to win one of five $200 Countdown vouchers.

- Liver disease
- Lung disease
- Heart disease
